Upper Airway Microbiota Relates to Season and Asthma Exacerbations

چکیده

Seasonal variation in respiratory illness and exacerbation pediatric populations with asthma is well described but poorly understood. We hypothesized that nasal microbiota covaries seasonal clinical trends season-specific microbial-host interactions during influence risk of exacerbation. Bacterial (16S rRNA) fungal (ITS2) profiling was performed on longitudinally collected secretion samples from children periods pulmonary health (Baseline, n=94 samples) their first subsequent (n=97). Data were analyzed conjunction extant transcriptional modules using R. Generalized linear (mixed) models utilized to identify differences incidence or composition (P<0.05). Exacerbation events co-varied similarly season [Generalized Additive Mixed Models (GAMM) P=0.039, P=<0.001, respectively]. So too did upper airway bacterial Model (GAM) P=0.001 baseline, P=0.016 illness] (GAM P=0.006 illness) detection Human Rhinovirus P=0.012 Baseline, P=0.003 illness). Airway closure on imaging relates to airway hyperresponsiveness and peripheral airway disease in asthma.

The regional pattern and extent of airway closure measured by three-dimensional ventilation imaging may relate to airway hyperresponsiveness (AHR) and peripheral airways disease in asthmatic subjects. We hypothesized that asthmatic airways are predisposed to closure during bronchoconstriction in the presence of ventilation heterogeneity and AHR. Fourteen asthmatic subjects (6 women) underwent c...

Inflammation and Airway Microbiota during Cystic Fibrosis Pulmonary Exacerbations

BACKGROUND Pulmonary exacerbations (PEx), frequently associated with airway infection and inflammation, are the leading cause of morbidity in cystic fibrosis (CF). Molecular microbiologic approaches detect complex microbiota from CF airway samples taken during PEx. The relationship between airway microbiota, inflammation, and lung function during CF PEx is not well understood. OBJECTIVE To de...

Reduced Baseline Airway Caliber Relates to Larger Airway Sensitivity to Rostral Fluid Shift in Asthma

Background: We have previously shown that when asthmatics go supine, fluid shifts out of the legs, accumulates in the thorax, and exacerbates lower airway narrowing. In the retrospective analysis of our previous work presented here, we test the hypothesis that the sensitivity of this process relates inversely to baseline caliber of the lower airways. Methods: Eighteen healthy (six women) and si...

Aspirin-sensitive asthma and upper airway diseases

BACKGROUND Aspirin exacerbated respiratory disease (AERD) consists of nasal polyposis, rhinosinusitis, asthma, and aspirin (ASA) sensitivity. OBJECTIVE This article details how to diagnose and treat AERD and describes the procedures associated with ASA challenge and desensitization, benefits associated with ASA desensitization, and appropriate doses for treatment. METHODS Criteria for diag...

Airway impedance entropy and exacerbations in severe asthma.

Variability of peak flow measurements has been related to clinical outcomes in asthma. We hypothesised that the entropy, or information content, of airway impedance over short time scales may predict asthma exacerbation frequency. 66 patients with severe asthma and 30 healthy control subjects underwent impulse oscillometry at baseline and following bronchodilator administration. On each occasio...
